surname; (2) regimental number (if ex-imperial, the unit); (3) condition necessitating admission; (4) date of admission day of month only; (5) the signature, to include the number of the hospital. As an example, the wire should read: "John Smith one two three four five six osteomyelitis tibia admitted sixteenth (Sgd.) Brown in charge of Fortyone."

(Hospital Circular No. 165) PERSONNEL INFORMATION

DECEMBER 19, 1923.

In order to relieve the medical officers in charge of hospitals of all unnecessary report work in regard to personnel data required by central office, a new form, Personnel by Designation or Position, has been devised, to be used in lieu of Monthly Personnel Report, Form No. 2607-a. This new form, together with sections X and Y, New Appointments, and Monthly Changes in Personnel, will hereafter constitute the Monthly Report of Personnel.

By reference to above-mentioned new form, copy of which is attached, it will be noted that columns have been provided for a report by number and by bureau designation of all personnel "on roll at end of last month" and "on roll at end of this month," and further classified by "in-patient" and "out-patient." Where an employee performs work in both the in-patient and out-patient departments of the hospital, such employee shall be entered under the department in which he performs his principal duties. Additions and separations as called for are self-explanatory.

In addition central office requires the record of personnel according to the "type of service" and "in-patient" and "out-patient" as indicated on the new form, Personnel by Designation or Position. It is therefore directed that you submit to central office, attention planning, statistics and budget division, the name, basic annual salary grade (annexing the letter B if the employee receives bonus), allowances, bureau designation in order as shown on the new form, marital state, ex-service status, claimant of bureau or non-claimant, whether, performing in-patient or out-patient work, as of the close of business, December 31, 1923. After this information is on record in central office it will not be necessary to forward the complete list of personnel each month as heretofore, but it will be very important that the information given on sections X and Y agrees with the number of additions and separations noted on the new form, Personnel by Designation or Position.

In reporting new appointments in section X, list carefully all information requested, according to each column. In the second column indicate the service to which the employee is assigned, namely, dental, dietetic, etc. Indicate by one star all new appointees assigned to out-patient work.

Note carefully all changes in section Y. If an employee is transferred to another type of service within the hospital, indicate same in the last column. Also note any change in the in-patient and out-patient work.

A supplemental code for hospitals covering bureau designations not listed in previous code No. S. A. 114 is inclosed.

The above information should be submitted so as to be available in central office not later than January 8, 1924. FRANK T. HINES, Director.
