examination performed by a military
flight surgeon or aviation medical exam-
iner. The report of medical examination
on Standard Form 88 will be forwarded
by the examiner to a designated aviation
staff medical officer of his respective
service for review and evaluation. If the
prescribed medical standards are met,
the reviewing officer will endorse the re-
port and prepare and issue to the quali-
fied applicant a CAA Medical Certificate,
Form ACA-1004. The endorsed report of
medical examination and the carbon
copy of the medical certificate issued will
be submitted through appropriate chan-
nels to the Chief, Medical Division, Civil
Aeronautics Administration, Washing-
ton 25, D. C., for file.

§ 406.13 Airman certificates.
(e) Mechanic certificate.

(2) Application. A single application
for this certificate shall be made on an
"Application for Mechanic, Repairman,
or Parachute Rigger Certificate or Rat-
ing," Form ACA-363.

a Form ACA-500. This form will con-
tain three parts: Part A, "Certificate of
Registration"; Part B, "Application for
Registration"; and Part C, "Bill of Sale."
An applicant for a certificate of regis-
tration shall mail the original and a
duplicate of Part A, an original of Part
B, an original of Part C or another bill
of sale or other form of conveyance de-
scribed in § 501.4 (b) (2) of this chapter,
and a registration fee of $4 to the Admin-
istrative and Records Branch of the Ad-
ministration in Washington, D. C. If a
contract of conditional sale is submitted,
an additional fee of $4 shall be enclosed
to record it. The applicant shall retain
the duplicate of Part B in the aircraft
as a temporary registration pending re-
ceipt of the certificate of registration,
Part A of Form ACA-500, and in the
event later notification from the CAA
authorizes continued temporary opera-
tion of the aircraft pending receipt of
such certificate of registration, the appli-
cant shall also retain such notification
in the aircraft.

(3) Issuance. This certificate will be

istration," Form ACA-500, Part A.

(e) Aircraft airworthiness certificate—
(1) Purpose. This certificate is issued
for an aircraft which complies with the
airworthiness requirements of the Civil
Air Regulations (Subchapter A of Chap-
ter I of this title). (In addition to an
airworthiness certificate, the Civil Air
Regulations require that there be carried
in the aircraft: An "Operation Limita-
tions", Form ACA-309; or an approved
airplane flight manual which sets forth
the limitations for safe operation; or
placards or listings or combination of
both, containing operating limitations,
insofar as they may have been prescribed
by the Administrator for a particular
aircraft.)

§ 406.16 Air agency certificates.

§ 406.17 Air navigation certificate
and notices. The following air naviga-
tion certificate will be issued by the Ad-
ministrator to qualified applicants. In
addition, the following air navigation
notices are required for the construction
or alteration of certain structures and
landing areas.

(a) Authorization to operate true
light-(1) Purpose. Upon certification
by an applicant that a light will be estab-
lished, maintained and operated as an
aid to air navigation in accordance with
the applicable requirements of the Ad-
ministration, the Administrator will is-
sue an authorization to the applicant
authorizing him to operate such light as
a "true light."

(2) Application. The application
shall be submitted on Form ACA-114,

"Certification and Lawful Authority to

Operate a True Light."

(b) Notice of construction or altera-

tion (1) Purpose. The purpose of this

notice is to advise the Administrator of

the construction or alteration of certain

structures and landing areas along or

near civil airways for which a notice is

required under Part 625 of this chapter.

(2) Submittal of notice. Notice shall

be submitted to the Administrator in ac-

cordance with the provisions of Part 625

of this chapter on Form ACA-117, "No-

tice of Construction or Alteration."

§ 406.19 Crew member certificate-

(a) Purpose. This certificate will be

United States citizens employed by

scheduled air carriers as flight crew

members or crew members on U. S.-

registered aircraft engaged in scheduled

international air transportation, as pro-

vided in Annex 9, as amended, to the

Convention on International Civil Avia-

tion.

(b) Application. An application for

this certificate shall be made on an "Ap-

plication for Crew Member Certificate,"

Form ACA-2116, and submitted to the

Civil Aeronautics Administration Air

Carrier Agent in charge of the air car-

rier by whom the applicant is employed.

(c) Issuance. This certificate will be

issued on a "Crew Member Certificate,"

Form ACA-2116.1.

(d) Surrender. This certificate will

be surrendered for cancellation by the

air carrier or the certificate holder to the

nearest CAA Aviation Safety Agent upon

termination of the holder's assignment

in scheduled international air transpor-

tation with the air carrier.
