Item 11b (2). Report rental payments for use of production machinery such as lathes, printing presses, packaging machinery, etc. Report rentals for office equipment associated directly with plant operations and located at the same site as the plant. Include rents for computers whether used for accounting and payroll purposes or used as production control equipment. Include rentals for transportation equipment whether for use within the plant site (trucks, tractors, power lifts, railroad engines, cars, etc.) or used for transporting materials or products into or out of the plant. Include rentals for use of trucks, tractors, railroad flat and tank cars; automobiles, and airplanes (whether used for freight or executive and sales personnel of the plant).

Item 12-Supplemental Labor Costs Not Included In Payrolls.

Item 12a. Legally Required Expenditures.-Report payments for all programs required under Federal and State legislation such as: Federal Old Age and Survivors' Insurance; unemployment compensation; workmen's compensation. Establishments located in Rhode Island, California, New York and New Jersey should also include State temporary disability payments.

Item 12b. Payments for Voluntary Programs.-For programs supported by joint employer-employee contributions, include only the employer payments. Include employer payments: (a) of insurance premiums on hospital and medical plans, (b) of life insurance premiums; and (c) of premiums on supplementary accident and sickness insurance. In reporting payments for insurance report net payments, i.e. gross payments less any offsetting dividends, refunds, or other reductions in premiums.

Include also payments or allocations: (a) on all pension plans regardless of method of administration, (b) on supplemental unemployment compensation plans, (c) on welfare plans, (d) on stock purchase plans in which the employer payment is not subject to withholding tax and (e) on deferred profit sharing plans.

The items listed in the paragraph above are generally considered as deferred rather than current income to employees and are not subject to withholding of income tax. If the payments or allocations by the company are made to a fund, payments to employees out of these funds should not be reported. However, payments made directly to retired employees or their survivors that do not pass through a fund should be included in Item 12b.

Exclude expenditures not of the type listed above including losses on company operated cafeterias and snack bars, cost of in-plant medical services, cost of free parking lots, discounts on employee purchases, cost of uniforms and other work clothing supplied to employees, and similar expenditures.

Exclude payments which should be reported as wages and salaries in Item 3 Payrolls, such as payments for holidays, vacations, sick pay, bonuses, unused vacation, jury pay, night shift bonus, suggestion awards, and the payment on profit sharing and special stock purchase plans which are subject to withholding tax.

If any of the items are maintained in your records only at the company level, allocate to the manufacturing establishments. In such cases, you may distribute the

total on the basis of the ratio of the payroll of each manufacturing establishment to the total company payroll unless you have developed your own method of making such allocations. Specify the method used and the approximate portion that has been allocated in Item 14 "Remarks."

Item 16 Certification.-Enter the month and day of the beginning and end of year this report covers. If a calendar year report: "From January 1 to December 31;" if a fiscal year, specify which (such as, "From November 1 to October 31").

F. PUBLICATION OF STATISTICAL RESULTS OF THIS SURVEY

Data compiled from the Annual Survey of Manufactures establishment reports for each year covered will be published in a series of individual subject reports. A bound volume containing all the report tables, together with a full explanatory text, additional product data compiled from the "Current Industrial Reports," and technical appendixes will also be issued.

The reports will provide the following:

1. General statistics (employment, payrolls, manhours, value added by manufacture and capital expenditures) for selected industries and industry groups and geographic divisions, States, large standard metropolitan statistical areas and large counties by major industry groups.

2. Value of shipments for classes of products (quantity of shipments data will be provided for selected classes).

3. Expenditures for new plant and new equipment at manufacturing establishments in operation and under construction for industry groups and States.

4. Value of inventories of finished products, workin-process, and raw materials for industry groups and industries.

5. Fuels and electricity data for industry groups and and States.

6. Value of assets and rental payments for selected industries, States and large standard metropolitan statistical areas.

7. Supplemental labor costs

8. Special Reports on acquisitions by manufacturing companies.

In item 9 of each Form MA100 the Census Bureau has preprinted the product class codes and description of the products reported by the establishment in the 1963 Census of Manufactures or 1964 Annual Survey of Manufactures. Please review the printed descriptions and codes against the list of products in this manual. Correct the codes and descriptions in item 9 if missing.

How to Choose the Appropriate PRODUCT CLASS in this List

Examine the title of each of the lines of activity described in the Table of Contents, Page iii. Select the page reference for the product listing describing the products made at your establishment. Each of the lists has been arranged to include, most of the products manufactured by establishments engaged in the specified major lines of activity.

To further clarify the content of each of the 5-digit product classes, the reference list includes examples of individual products of the product class and cross references to related products classified elsewhere. Group your products and report values by each of these 5-digit product classes. A single value may be reported for all products included in one product class; however, products included in separate product classes should be reported separately. Do not combine values for products included in different product classes.

If you are unable to identify or classify your product in this reference list, enter its description and value in Item 9. To enable us to classify such product entries, detailed product descriptions should be entered: for example, an entry "household washing machines" can be coded, but "household appliances" is too broad for classification.
